We're immediately interviewing Medical Social Worker candidates for a highly-rated Home Health and Hospice agency in beautiful verdant Provo, Utah. 


In this position, a Medical Social Worker will provide social work services to meet the needs of their patient and family; they will communicate with the patient and assess their emotional factors, counsel the patient in long-term and short-term planning, assist in Hospice orientation and discharge planning, and comply with legal and ethical obligations regarding the patient's well-being. 


- Schedule is 9-5 Monday - Friday (with some flexibility).

- Office from your home, and visit patients in their homes on your schedule.

- Pay is highly competitive.

Applicant Expectations:

- Certification or licensure in the state where services will be provided and conducted IS REQUIRED.

- A Master's degree in Social Work IS REQUIRED. 

- LCSW certification is preferred, but we will consider candidates who are working toward their certification.
